We’re evolving. Beginning January 1, 2026, LiveWellLocal.org will retire and many of its programs are in the process of being integrated into Findhelp, a recently launched platform at Providence for direct social care organizations. Providence Findhelp will continue the work of LiveWellLocal.org by helping connect community-based resources to those in need. It offers referral tracking, expanded translation support services and resources, and a growing network of community partners and services. More information on this transition is below.

Why did LiveWellLocal.org transition to Providence Findhelp?
LiveWellLocal.org has transitioned to Providence Findhelp. Providence Findhelp now serves as the central social care resource and referral hub for patients and the community, offering expanded capabilities to better support our shared goals.
What does this mean for community members?
Community members can now access the Providence Findhelp platform, where many LiveWellLocal programs are in the process of being integrated. This ensures continued access to vital community resources. Providence patients can also access Findhelp directly through their MyChart account, making it easier to connect with support and services.
What does this mean for community partners?
Providence Swedish Caregivers who screen patients for social drivers of health, can now make a direct referral (warm handoff) to local a Community-Based Organization (CBO) who is listed in Providence Findhelp and who has claimed their listing.
Most community partners who met the Providence Findhelp qualifications (direct service providers) and were previously listed on LiveWellLocal.org are in the process of being transferred to Providence Findhelp. - Open Access means anyone can anonymously search for or create free accounts, including access to tutorials and training
- LiveWellLocal resource listings are being transferred to Providence Findhelp, where you can claim and continue to manage/edit your listing(s)
- Language support is expanding to ensure accessibility for all
- Providence branding will be embedded for consistency and trust
- Referral Tracking to manage, monitor and update incoming and outgoing referrals
- Better data for informed decisions, such as free reporting dashboards
