Health Equity

Why Health Equity Matters

Understand more about how social determinants contribute to our health behaviors:

The leading causes of disease and rising healthcare costs are chronic diseases. They are largely preventable or can be successfully managed. However prevention and success rely not only on healthy habits of individuals, but also on the daily conditions, environments and experience of our patients, clients and the community.

“This country will not be a good place for any of us to live, unless we make it a good place for all of us to live in.” – Theodore Roosevelt

Our purpose here is to provide collaborative linkages to health equity tools, trainings and resources, promote local opportunities to advance equity and build connections with others who have common goals.
