The leading causes of disease and rising healthcare costs are chronic diseases. They are largely preventable or can be successfully managed. However prevention and success rely not only on healthy habits of individuals, but also on the daily conditions, environments and experience of our patients, clients and the community.

Our purpose here is to provide collaborative linkages to health equity tools, trainings and resources, promote local opportunities to advance equity and build connections with others who have common goals.
Defining Health Equity
Health Equity: Attainment of the highest level of health for all people. Health Equity means efforts to ensure that all people have full and equal access to opportunities that enable them to lead healthy lives. Learn more about a Health Equity Framework (SFSU)
